choco Community Clinic will treat any person(s) regardless of their ability to pay. When you visit the Clinic, you may be asked to verify your annual income. This can be done by showing your tax return or other financial documentation to the receptionist on your first visit. If you don’t have any of these materials, don’t worry. We will find a work-around so we can help you while still offering steep discounts in providing medical services.

First, discounts are based on a sliding-scale depending on your level of income in relation to the federal poverty level adjusted for family size. In 2003, the poverty levels for individuals and families are:

  One person: $8,980
  Two persons: $12,120
  Family of 3: $15,260
  Family of 4: $18,400
  Family of 5: $21,540
  Family of 6: $24,680
  Family of 7: $27,820

This means that if you (or your family) fall into one of the categories above, you will pay a minimum of $25 for a visit to the Clinic. If your income is over the poverty level but less than twice the poverty level, we will adjust your fee in accordance with your ability to pay.

All fees includes a visit with a physician, nurse or other licensed medical professional, and treatment. Treatment can include such procedures as immunizations, casting, care for cuts, the flu, prenatal care and so forth. Click here to see the scope of services we provide.

Second, if you have third party coverage such as auto insurance, employer-sponsored health insurance, individual health insurance, Medicare, Medicaid, Workers Compensation and so forth, your insurance will pay Ochoco Community Clinic directly. You will be responsible for paying only your deductible and co-pay amounts to the Clinic.
