At Ochoco Community Clinic, we want to give you the

in our services.
When a patient(s) visits the Clinic, sends us e-mail or completes
our on-line form, we are required by Oregon law
to protect and safeguard any information you
give us unless you sign a consent form
that permits us to speak with other medical professionals
outside Ochoco Community Clinic. In that event,
we will discuss only the aspects of your case
that are pertinent to your treatment or treatment
plan.

if you complete our on-line
form, we will keep your Internet e-mail
address, phone number, street address, questions
and
all other information you provide strictly confidential.
We will never share it with an outside
party. This is our policy, and this is our
promise to you.
